INFORMATION We
kindly request that new patients arrive 10-15 minutes before
their appointment to fill out a questionnaire on personal and
medical history.
Insurance: We also realize
that insurance coverage can be difficult to understand, when
scheduling an appointment you will be asked for your insurance
information. Please have your card available. We participate
in many managed care plans. Please do not hesitate to ask us
about one you are involved in or considering.
Referral Policy: Insurance
referral forms provided by your family practitioner are
required at the time of your visit. You will need to
reschedule if you do not have this
information.
Financial
Policy: Payment of services not covered by
your insurance plan such as visits, deductibles and/or co-pays
are required to be paid at the time of you visit. Our staff is
available to answer you questions regarding
